Sign Up to Help with a Birthday Boost Box

Welcome! You can find our current birthday box requests here: signup.madisonbirthdayboost.org.
 

Each row represents one child’s birthday, and you can sign up to provide specific parts of their celebration box.

💡 How It Works:

1. Review the “Sign Up” tab.

2. Use the filters to find the locations or items you want to sign up for. You can sign up for an individual item or a whole box.

3. Drop off or ship your items to the Preferred Location by the due date.

4. Check the sign-up form for allergy info or special instructions.

📦 What Can I Sign Up For?

• Tableware
  Napkins, plates, cups, and a plastic tablecloth (for 8 people)
  
  

• Cupcakes
  Usually 12 cupcakes + candles (homemade or store-bought; check allergy info), but families can also request cupcake mix/frosting or Oreos
  
  

• Activity/Game
  Something fun for the kids to do (bubbles, BINGO, decorate-a-mask, etc.)
  
  

• Decorations
  Balloons, crepe paper, ribbons, tape, and optional theme items
  
  

• Party Favors
  6 small goody bags (stickers, trinkets, etc.)
  
  

• DIY Decorations
  Handmade banners, posters — include the child’s name and theme
  
  

• Gift + Card
  Unwrapped gift, gift bag or wrapping paper, and a card for the parent to give to the child
  
  

• Box Decoration
  Pick up a box from us and decorate all 4 sides

✅ Tips for Success

To be alerted about new sign ups: 

1. Go to the My Signups tab. 

2. Enter your email. 

3. Click Notification preferences to be notified of new sign ups.
